To make the process as smooth and efficient as possible, we provide the most important documents as downloads right here on our website.

Submit your request for services on-line using the "Request Services Now Tab" on this website or call the office  at 678-782-7272 to complete a referral.  Schedule your appontment and then "ASK" for the code to access our INTAKE PACKET on-line so that you may download the form.   Enter the agency code and select the appropriate download link and return the completed documents to our practice either in person, by fax (866-936-6574" or as a scanned PDF file via e-mail to apexfamilyservices@mail.com. Naturally, all forms are also available at the practice; however, filling them out in advance saves time.
